Course Summary
Patients sometimes decline the evaluation and treatment recommendations that a provider believes are necessary for optimal care. This can lead to challenges both in documentation and in the relationship that patient has with the healthcare system. Traditionally, providers have used the “Against Medical Advice†form, AMA, to document a patient’s refusal to follow a care plan. However, a newer tool, the “Informed Refusal†presents an option that may be appropriate in some situations to facilitate provider-patient shared decision making and to create a less adversarial climate around the care plan. This course presents real case studies where the issues around appropriate use of AMA and informed refusal are discussed. Upon completion, providers will have understanding of how these tools may be an effective strategy to optimize patient safety and to reduce allegations of failing to adequately inform patients who decline care of potential adverse outcomes.

-Enumerate the elements that must be met for informed refusal. -Provide best practices for the documentation of informed refusal. -Compare and Contrast the benefits and risks of AMA vs. informed refusal. -Prioritize the clinical situations by Risk profile in which an informed refusal form should be completed.
